ltmodem/ltserial are for pre 2.6.12 kernels.
For 2.6.13 use the martian driver from
http://linmodems.technion.ac.il/packages/ltmodem/kernel-2.6/martian
You did not send a ModemData.txt so that your decision that your modem
requires the ltmodem series driver (martian for kernels above 11) is
YOUR responsability, not ours.
Knowing that the chipset comes from Lucent/Agere is NOT sufficient to
decide that this is right driver.

(when they try to start pppd) write that

-- Exit status of pppd 16
