[PATCH] VMX: Properly return error to userspace on vmentry failure

The vmexit handler returns KVM_EXIT_UNKNOWN since there is no handler
for vmentry failures. This intercepts vmentry failures and returns
KVM_FAIL_ENTRY to userspace instead.
