[PATCH 1/6] qemu-kvm: Introduce bit-based phys_ram_dirty for VGA, CODE and MIGRATION.

Replaces byte-based phys_ram_dirty bitmap with 
three bit-based phys_ram_dirty bitmap.
On allocation, it sets all bits in the bitmap.
