How about setting up a kdump for the host, triggering a crash dump and investigating that? If it's a write owner forgetting unlock, rw_semaphore owner field could point to it. But it can also be that a down_write() is pending, blocking further down_read()'s, while the current reader is missing and not by design accurately tracked in the owner field.
