The (!pua) failure is handled in tce_iommu_build_v2() from
drivers/vfio/vfio_iommu_spapr_tce.c as:

The (!mem) can succeed if the container is in VFIO_SPAPR_TCE_IOMMU mode
(not VFIO_SPAPR_TCE_v2_IOMMU). Remember that the userspace can call
ioctl(vfio_kvm_device, KVM_DEV_VFIO_GROUP_SET_SPAPR_TCE) without having
memory preregistered so tables will appear in the
kvmppc_spapr_tce_iommu_table list in KVM.
