[PATCH 16/18] MIPS: KVM: Use MIPS_ENTRYLO_* defs from mipsregs.h

Convert KVM to use the MIPS_ENTRYLO_* definitions from <asm/mipsregs.h>
rather than custom definitions in kvm_host.h
