[PATCH v2 57/60] KVM: PPC: Book3S HV P9: Remove most of the vcore logic

The P9 path always uses one vcpu per vcore, so none of the the vcore,
locks, stolen time, blocking logic, shared waitq, etc., is required.

Remove most of it.
