These have all been applied to kvm-arm-master, with the fixup rebased,
a minor comment adjustment, and a reword of the long patch title.

The vgic edge-level fix has also been applied.

Please note that patch 6 breaks qemu compatibility, and I don't
believe this has been addresses. I will send out a separate patch.
