NASA INVESTIGATION BOARD ISSUES FINAL REPORT

An investigation board, named to look into the circumstances 
surrounding the March 17, 2006, construction worker fatality at 
Kennedy Space Center, has issued its final report.

The board was chaired by John Casper, manager of the Space Shuttle 
Management Integration and Planning Office at NASA Johnson Space 
Center in Houston. The five-member investigation team examined the 
facts surrounding the incident, gathered evidence, determined 
probable cause and recommended corrective actions.
