Sounds reasonable.

Is this bad?

Okay. Quit Kmail, remove .kde4/share/apps/kmail/mail/.myfolder.index, 
manipulate .kde4/share/apps/kmail/mail/myfolder, and start Kmail again. 
Which will detect that the index is gone and re-generate it.
