The name of the icon must correspond to the MIME type or be a 'generic icon'.  In theory, it is possible to assign different icons using the 'generic icon' file, but KDE does not have a GUI to do this.
In your case, the best way would to give your icon (or a copy) the correct name and it will automatically be used.  To determine the name, take the file type at the top of the File Type setting dialog and replace all of the "/" with "-".
