How do I add item to the desktop root menu ?

Hello,

How do I add item to the desktop root menu ? I don't mind
editing some configuration text files for that. (By root
menu I mean mouse right-click on the desktop background).
Failing  that, how do I add items under "Create New" under
root menu ?
