Which actions appear and at which position can be configured _if_ the developer uses this part of the KDE functionality. If it is used, a file usually called applicationname.rc is in the .kde/share/apps/applicationname directory - Open With is almost certainly created by the KDE libs, never in application code - freedesktop.org or rather any work group there, has not created a specification for file type associations yet.
