My guess is that the answer is "with great difficulty". IIUC, the menu is
updated by kbuildsycoca, which is run at KDE startup and whenever you
change certain options (like file associations). Stopping that running
could be tricky. Please explain more fully what you're trying to achieve,
and we can perhaps suggest other ways of doing it.
