Usually the only way this could happen is with a faulty kdebase/kdelibs in the upgrade packages (or upgrading from within kde) but since one upgrade was successful then that is not the case. Its possible that you lost that sector of the disk in a crash or hard-boot but what you must be missing are the files in $KDEDIR/share/mimelnk/inode. KDEDIR is /opt/kde3 for suse or /usr for RH/Mdk etc. You seem to be missing all the *.desktop files from that folder.
