Have you tried creating a 'Junk' folder (make it maildir) to file spam into and setting it to expire read messages after a day or so? KMail ought to prompt you on exit to expire messages. Btw, KMail issues are best dealt with on the kde-pim mailing list, where at least one of the KMail devs usually posts.
