The insert_entries function was the function used to write PTEs. For the PPGTT it was "hardcoded" to only understand two level page tables, which was the case for GEN7. Akash pointed out that this change doesn't depend of 48-bit ppgtt, so it should be moved earlier in the patchset (before PML4 is introduced).
