But this didn't remove the odd folders nor the files in these folders.
Just remove them by "rm -rf" or move them to another location outside
the spool?

rm -fr if you are 100% sure you don't need the files. Move if not 100% sure .:-)

Since cyrus doesn't know about the folders in mailboxes.db it shouldn't care either way.
