I simply don't provide plain IMAP on the public interface of the server. 
In the SERVICE section of /etc/cyrus.conf comment out the imap line, and 
have:

imaps     cmd="imapd -s" listen="[xxx.yyy.zzz.www]:imaps" prefork=1
