Well, here's what I've found so far, regarding differences.

This current config, which is the RPM install of cyrus-imap, also includes an RPM install of squirrelmail.

My other RPM install of cyrus-imap, uses squirrelmail, but squirrelmail was downloaded, installed, and configured by me.  Perhaps this is where my problem lies.

autocreateinboxfolders was used a workaround while I tried to find the root of my problem.  A previous post that stated that autocreateinboxfolders was a patch to cyrus explained a good deal for me.

I'm confused by your description. In what way does SquirrelMail interact 
with Cyrus IMAP that would result in permission denied errors? If you 
have a valid user that authenticates, and your prefix is set up 
properly, SquirrelMail should be able to create the necessary folders. 
Look for differences in SquirrelMail's config.php file on your working 
Centos setup. Using autocreate with Cyrus seems a red herring here. It's 
anybody's guess what's wrong with Outlook. I *always* test/troubleshoot 
that beast last.
