There is a difference between backward accountability (blame assignment) and forward accountability (making sure the mistake is not made again, which implies understanding the mistake and why it was made in the first place).  I agree the latter has value.

Not speaking to the specifics of this case, I would argue that sometimes the former done without animus (or with as little as humanly possible) can help foster the latter.

All people make mistakes.  Sometimes, we can make "blame" less pejorative a word and still use it.
