I suspect that what You have is an FW issue....

See this lines from Your debug:

2009/09/17 20:45:49.199 3             RasSrv.cxx(250)   RAS     Send to
200.96.170.116:1719
disengageRequest {
     requestSeqNum = 9
     endpointIdentifier =  10 characters {
       0037 0031 0030 0030 005f 0050 0072 006f   7100_Pro
       0078 0079                                 xy
     }
     conferenceID =  16 octets {
       02 2e 9d 22 ce df ca 10  1d 94 d5 ea db 78 51 b7   ...".........xQ.
     }
     callReferenceValue = 21193
     disengageReason = forcedDrop <<null>>
     callIdentifier = {
       guid =  16 octets {
         02 2e 9d 22 ce df ca 10  1d 93 d5 ea db 78 51 b7   ...".........xQ.
       }
     }
     gatekeeperIdentifier =  6 characters {
       0073 0061 0072 0067 0061 0073             sargas
     }
     answeredCall = false
   }
2009/09/17 20:45:49.200 5             RasSrv.cxx(264)   RAS     Sent
Successful

They inform that GK request the disengage after 15 seconds (and not the
30 that You reported) 'cause the call was not answered (answeredCall =
false).

So review all Your connection path and try to find were's the problem.

Just as a side note, using IPTables as FW base, I never had problems
with NAT, but You have to enable proxy support on Your config and is
desired that You EP support the highest H323 version... ;)
