To sum up your problem in 1 word NAT.
The internal gatekeeper needs to send the external address of the NAT to the external gatekeeper.
I have rectified this in the CVS for the next release of GnuGk so you can specify the external IP (or DDNS address) of the NAT box and the internal gatekeeper when sending out of the NAT will always show the correct external IP address (not 10.x.x.x). There is also an example configuration file nat.ini in the CVS
