This is a GnuGk issue, and have had a few enquiries on the list about it and it should also be applicable to OpenH323/Opal users wanting to use GnuGk
OK v2.2.4 does not support working behind a NAT correctly. I have since fixed that problem however you will need the CVS version to compile.
Now I have also updated the GnuGk Installer for Windows, It contains a configuration menu which allows you to set and change the port forwards in the Installer. (Note: There are a few you need to do so you need to write them down or you can just put the box in the DMZ of the router).
I have compiled the latest CVS snapshots of GnuGK, OpenH323 and pwlib and built a test installer for evaluation.

This is not the official next release, it is only a test installer for people wanting to test install GnuGk behind a NAT.
This version has postgres support but no mysql.
Some of the features of the installer
1. Installs GnuGk as a Service
2. Automatically configures the gk.ini from the installer (with options set in the installer)
3. Fully menu driven, can access from the startmenu
Telnet status port
Add Users (Program which inserts new passwords for users in INI)
Edit the INI,
Reload the gatekeeper
There is still a lot more which can be added later but it should be a good start.

My Internal gnugk box crashes when trying to have my gnugk box behind a firewall (Internal) try to register as an endpoint to a Gnugk Box on the external network which is registered to a cisco box as an endpoint. I am running 2.2.4 of gnugk gatekeeper software, openh323 V 1.18.0 with pwlib v1.11.0. This box is a Fedora 5 Box, and the external box is a Debian box. Is this a bug, configuration issue, or maybe a OS issue.
