The problem is that EP's cannot receive calls directly from behind a NAT, This is a well known issue.

You have a couple of alternatives.

1. Put a GNUGK on the inside of your NAT Box communicating with another GNUGK outside the NAT box and the inter gatekeeper communication will traverse the NAT for you.
2. Put the Nat'd EP on a fixed IP and manually set port forwards in the NAT box to forward port TCP 1720 and UDP ports 5000-5999 (OpenH323 based, my vary for some equipment) to the endpoint.
3. Put the Nat'd EP in the DMZ of the NAT box and all incoming traffic will be forwarded to it.
4. Use a NAT friendly softphone like PacPhone www.pacphone.com which can work behind NATs with GNUGK.

I had a similar problem may end point was under nat,the client side I was working with fast start but the end point was sending in lowstart it was behind a nat.

currently i fatch a problem,my network diagram is as follows.....

Gateway1 and gnugk are in under same isp but deferent subnet of public ip.
Gateway2 is in seperate isp with public ip.
 
when any one/end point call from dialup with public ip can get dialing tone and can talk.
 
from dhcp nated box when call going out by gateway1 no ringing tone
but when call going out by gateway2 then get r ing tone
 
when any one/end point call from private ip behind DHCP nat box from diffrent isp don't get ring tone/dialing tone but when remote end receive call then both end can get each other.so the dialing end can't get response his call is dialing or not.
