I assume that you are asking about a NAT router.
Yes, it will work, see NetworkInterfaces statement in
http://www.gnugk.org/gnugk-manual-4.html#ss4.2

Your public IP address should be static.  Otherwise,
you'll need to update the configuration file when it
changes, and the endpoints will need dynamic DNS to
find the GK.

You need to forward the proper ports (RAS, call signal,
H.245, RTP) across the NAT.

This feature did nor work correctly in some 2.2.x versions.
I don't know if it has been fixed.  If you have trouble,
try version 2.0.9 .
