1. Forward TCP port 1720 across the NAT to the GW.
2. Likewise for H.245 port(s), or avoid separate H.245
   connection by using fast start and/or H.245 tunneling.
3. If possible, also forward UDP ports used for RTP/RTCP.
4. Set H245Routed=1

At this point, it should be working.  It may be possible to
turn off ProxyForNAT, which can improve quality when the
originating phone and the GW are both distant from the GK.
