Why? Is not the RADIUS server smart enough to process
the milliseconds field when it is shorter than 3 digits?

No , It does not change value but If I want to take CDR or Accounting
from Quintum or Cisco and ofcourse from GnuGK to a Central RADIUS
server then I am in trouble as Quintum or Cisco give 3 digits value .
