It's simple - your RADIUS server can pass h323-redirect-number and/or h323-redirect-ip attributess in Access-Reply messages and GnuGk (through RadAuth/RadAliasAuth) will take the new number/ip from these attributes. What logic is behind a RADIUS server is up to you.
