Hello I see your problem and here I solved it by using vpn under tcp or udp...
that is:  
1) I have the endpoint in a windows box...
	I start a pptp to the gnugk machine and inside the vpn that is thus formed,
	everything works, and I do not have to deal with nat/port remapping/forward problem.
2) using any computer to set up a vpn to the gnugk machine  (windows, linux, freebsd...)
	and the gnugk machine opens a small network to the client... and then your quintum
	box can receive calls from the gnugk...

explicitally program the nat box (adsl modem) port by port to forward all needed, and so known ports
1719,1720,1721... to the quintum box... use an packet sniffer to see if the packets are
really reaching your quintum...
but some adsl modem has the firmware broken...
