I think you should add your public IP to the list
of NetworkInterfaces:
    NetworkInterfaces=192.X.X.202/32,192.X.X.241/32,123.123.123.123/255.255.255.255
Disable ProxyForNAT and NATed endpoints sections, 
as these settings do not take any effect here. Make sure the mahchine
gnugk is running on does not have any ports blocked.
I think your problem is that exterior endpoints receive private LAN
addresses.
