I've always added by opening the sub-menu I want to add something to, and using the 'right-click' options. Right click, choose "entire menu" / "Add new item to this menu"

Ugh that is incredibly counter-intuitive.

Surely there should be a more logical way to get to the 'start-here://'
point?
