Yes, a Google Form would be perfect.
For now, lest we get too ambitious with the scope and delay the
survey, I think we should limit the survey to simple
radio-button-checkable options for now, and leave the free-form-type
responses for another exercise. :)

It would be a great idea to have a form online somewhere that
people could submit their use cases and deployment details. We
used to have something like that, way back when.
What other things, besides what you've listed, would be useful
to know? What should we offer in return to those who fill out
