io-cache Bug?!

Hi Bryan,

Can you reproduce this issue in a simple client/server setup (without any
clustering translators, performance translators except io-cache)? How easy
is it to reproduce the bug? Is it possible for you to give a setup and
series of operations which can reproduce the bug consistently?
