+ rerere: fix an off-by-one non-bug

Code clean-up and minor fixes.

"git rerere" can encounter two or more files with the same conflict
signature that have to be resolved in different ways, but there was
no way to record these separate resolutions.

Originally merged to 'next' on 2015-05-28
Stop supporting "git merge <message> HEAD <commit>" syntax that
has been deprecated since October 2007.
Will keep in 'next' during the 2.6 cycle.
