I agree with everything you said in your review. As to the example index-v4 series, it needs to be stressed that the series did *not* happen in the order it was presented. While the series was being written, it looked pretty much "Ok, now we can read the header but nothing else works" kind of "implemetation steps". Only very few people can write a series in the right presentation order from the beginning.
