To quickly fix it you should replace in the file stylesheets/plainhtml.xsl the following line: <xsl:import href="http://docbook.sourceforge.net/release/xsl/current/html/autoidx-ng.xsl"/> with <xsl:import href="http://docbook.sourceforge.net/release/xsl/current/html/autoidx.xsl"/> (note there is no -ng after autoidx)
