Well, if your interfaces are not configured with static addresses as only DHCP is handled. Also it looks like that NetworkManager does not notice interfaces which showed up after it was started. Say, you plugged in a Cardbus card with a wireless on it. It is not clear to me if NetworkManager is expected to react but it does not without a restart or two. Likely if you are adding ipw3945 in rc.local you should also restart NetworkManager there and then everything will work.
