Make sure you have the "Exchange Operations" plug-in enabled.  Look in
Edit -> Plugins.  For some reason that plug-in likes to disable itself.

I don't know why "Microsoft Exchange" is even listed as a Server Type in
the Account Editor/Assistant if the plug-in is disabled.

Bingo... that was the problem.  Very confusing to a user that's going
through the wizard and couldn't have changed the plugin settings yet.
