This sounds like critical harddrive failure. Run smartctl -a /dev/sdxx|grep -i failed But do not take the output for granted. If your logs show/have shown sector errors, then the HD is probably corrupted and has taken random data with it. Of course, it could be the cables, or the controller as well.
