I would recommend replacing hand-written version in Source0 to use %{version}
also the second time.

Unless I am mistaken %{_libexecdir}/%{name}/ directory is not owned by the
package. At least %dir %{_libexecdir}/%{name}/ should be present.
Similarly %{_datadir}/c_icap/templates/squidclamav/ should be included also
with directory, not only all its files. Remove * at the end of that line.

On the other hand use %{_mandir}/man1/%{name}.1* instead of fixed .gz suffix
for a man page.

%{_sysconfdir}/httpd/conf.d/%{name}.conf needs Requires: httpd-filesystem to
have the directory owned.
