First one is a license issue, indeed, some parts come from docbook-xsl, and the license file is missing. I suggest using http://docbook.sourceforge.net/release/xsl/current/COPYING under a different name. Second one is that xsl/mathml2 is in fact derived from http://sourceforge.net/projects/xsltml/ Similarily, the license file is missing. I suggest using the README from the latest xsltml release, which is already in xsl/mathml2, renamed appropriately. I have checked that the differences are only in entities.xsl, upstream should certainly be contacted such that this is fixed and both versions converge. There are also additional files in dblatex, namely mathml.xsl and mapmmlent.xml (which seems to be autogenerated, and it would be better if func2map that generated it was shipped).
