Just to be sure, is the bug number supposed to refer to the package
review ticket, or is a separate bug supposed to be opened that just
requests the branch?
What about packages that branched some time ago and are just now
getting a branch to an older release for whatever reason?

Good questions. Bug number should be of the review, and it is really
optional. It only makes it faster for the branch approver to be sure
the process was followed properly before branching.
