While I am certainly not in favor of more "Are we there yet?" emails, I wonder if a gentle reminder every few months might not be appropriate if there are open PR's, as, as you say, sometimes there are valid reasons. I also wonder if a PR open for more than, say, 18 months might trigger a check to make sure the packager is still active.
