Found part of the problem:

In /etc/firewalld/direct.xml:

<rule priority="0" table="filter" ipv="ipv4" chain="INPUT">-p tcp -m multiport --dports ssh -m set --match-set fail2ban-sshd src -j REJECT --reject-with icmp-port-unreachable</rule>
---

But even after fixing that to f2b-sshd, if "ipset list" is empty I still get the error.
