Isn't that a bit of an overstatement? Realistically, if you install FC4 later than on the day it is released you'll likely want to do a "yum update" pretty much immediately. If Extras is enabled by default in Yum (and any other package managers that are deemed to matter), this will also pull in the updates for anything that was moved into Extras. Sure, that's a two-stage upgrade (anaconda + yum), but with the amount of package updates that Fedora has been seeing, it basically is anyways. >From what I have understood it sure sounds like the Extras repo is supposed to be enabled by default?
