The segfault sounds similar to

The problem with the card I had was that the raid appeared with a very
long name that changes from boot to boot, with the consequence that, as
well as the segfault, the initrd referred to a a raid id that was no
longer valid, and the system booted off half of the raid, breaking the
mirroring.
The segfault was fixed in RHEL/Centos 5, and I worked around the changing
name by hacking the initrd.img .
