hardware error or fs corruption?

Hi Mohammad!

This looks like an UDMA/Harddrive failure. Try disabling udma on the drive
(hdparm -d0 /dev/hdxxx), if the errors still occur your drive has grown
some bad sectors, replace it ASAP.
