This may be relevant: The crash does not happen if I run the HDMI monitor at 30hz. This means the monitors can be run at different refresh rates. The DisplayPort monitor does not support 59.94hz or 50hz. My conclusion is that the crash occurs if the HDMI monitor is set to a refresh rate that the DisplayPort monitor does not support. And that explains why the HDMI monitor works on its own. With only the HDMI monitor connected, all the connected displays support 59.94 so everything is fine. I did try adding 3840x2160 59.94 to the displayport monitor via xrandr to see if I could run both displays at 59.94, unfortunately it does not seem to support it and I just get a black screen.
