[PATCH for-3.15 2/3] dm era: support non power-of-2 blocksize

The dm-era target is expected to be paired with the dm-cache target
which has support for non power-of-2 data blocksize.  So dm-era should
also support an origin data device blocksize that is non power-of-2.
